LCD Data pins 0-3

Why don't we ever use lcd pins 0 to 3?

You do, if you use 8bit mode.
4bit mode only uses the top four pins, Read the LCD data sheet. Key thing is that LCD's are _slow_. Something like a clear screen command (for example), takes 40mSec to execute. 4bit mode adds a couple of uSec to the communication, and saves four pins on the PIC. You can drive the display with just 6 pins from the PIC if you want. Given that in most cases saving pins is more important than the tiny extra time, the standard drivers use 4bit mode.
