Keep blowing my Microchip MPLAB ICD-2

I am noticing that the ICD2 (from Microchip) gets blown easily, i've damaged 3 the past 5 months.

Somehow, the 9V dc adapter fails to power up the unit. In order to make it work now, I then have to go get my 12V from the USB port, and the 5V I have to plug my target unit's power on.

Is anyone else having a problem with blown ICD2s?

Maybe its the MPLAB software setting.

Sounds like maybe your telling the software to try to power the test board throught the ICD. (which is the +5 usb volts or the +9V adapter voltage regulated to +5)

AND

also powering the test board it self.

It should be ONLY powered through the ICD
OR
ONLY powering the test board itself.

not both

BTW. if your getting +12 from a USB cable, THATS WRONG.
should be +5 from shield to pin1.

I have 5 and have had no problems with any of them (been used quite a bit over the last couple years).

I am on my 4th unit. the last one was hooked to a f74 that has been in use for 2 years it was not a development board it stopped working., so no wrong connetions. good thing microchip replaces them.
